In 1986, a severely injured boy with a gunshot wound to the head is brought to a hospital in Dusseldorf, West Germany. Ignoring his superior's orders, Japanese surgeon Dr. Kenzo Tenma saves the boy using his exceptional surgical skills. The communist world is engaged in a sinister conspiracy to overthrow West Germany. Czechoslovak secret police officer Franz Bonaparta and others raise children to become fighters sent to capitalist countries, giving birth to a monster named Johan. The monster within Johan grows larger, leading him to murder those around him. Like the protagonist of Franz Bonaparta's picture book "The Nameless Monster," this creature possesses people and gradually devours those close to them. Years later, in 1995, Johan has grown into a massive monster and ruthlessly kills one of Tenma's patients. As if something bursts inside him, Tenma begins to pursue the monster Johan. Can Dr. Tenma stop Johan's killings? What exactly is the picture book "The Nameless Monster" that stimulates Johan? Why does Johan continue to kill? The story unfolds in Germany and the Czech Republic after the fall of the Berlin Wall.
